CHECKOUT PROCEDURES
1. Items may be checked out for two weeks. Extended time may be arranged.

2. Individuals checking out items are responsible for picking up and bringing them back to ACES or paying for postage both ways.

3. Items borrowed will be returned in the same condition as when loaned. Let ACES know immediately if something is missing or wrong with the item.

4. Should an item be torn up or lost, schools or individuals will be responsible for replacement cost.

5. Checkout will be on first-come basis.

6. Items may be checked out by mail, telephone, or e-mail.

TO SCHEDULE TESTING:
1. Send an e-mail  along with the Request for Diagnostic Services Form to the diagnostician. Be sure to include date parents signed permission to
test and let the diagnostician know if the parents did not waive the 10 day waiting period.

2. The diagnostician is also available for consultation, eligibility staffing, and IEP meetings if needed.
